GOAL
To reduce recurrent hospitalization by assisting patients in finding permanent housing, care coordination, benefit acquisition and a multidisciplinary team of healthcare providers within a short, transitional time frame.
ELIGIBILITY CRITERIA
KERN COUNTY

Admission paper work has to be coordinated with the center from the institution of transfer prior to admission.

Residents are authorized by the State to stay a week at a time, complete stay cannot be longer than 30 days.
​

Assistive devices are required to be set up in advance.
​

Residents must be able to self mobilize household distances and use restrooms and showers without mechanical assistance.
​

Residents should be able to self administer medications, with help of health literacy support staff and ancillary team.
​

Medications are stored in locked secure setting and to be received under staff supervision.
​

On-site staff supervision is provided 24/7.
TULARE COUNTY

Residents are authorized by the State to stay 30 days with extensions in 30 day increments if medically necessary. There is a 90 day limit.
