GOAL

To reduce recurrent hospitalization by assisting patients in finding permanent housing, care coordination, benefit acquisition and a multidisciplinary team of healthcare providers within a short, transitional time frame.

ELIGIBILITY CRITERIA

KERN COUNTY

  • Admission paper work has to be coordinated with the center from the institution of transfer prior to admission.

​

  • Residents are authorized by the State to stay a week at a time, complete stay cannot be longer than 30 days.

​

  • Assistive devices are required to be set up in advance.

​

  • Residents must be able to self mobilize household distances and use restrooms and showers without mechanical assistance.

​

  • Residents should be able to self administer medications, with help of health literacy support staff and ancillary team.

​

  • Medications are stored in locked secure setting and to be received under staff supervision.

​

  • On-site staff supervision is provided 24/7.

TULARE COUNTY

  • Admission paper work has to be coordinated with the center from the institution of transfer prior to admission.

​

  • Residents are authorized by the State to stay 30 days with extensions in 30 day increments if medically necessary. There is a 90 day limit.

​

  • Assistive devices are required to be set up in advance.

​

  • Residents must be able to self mobilize household distances and use restrooms and showers without mechanical assistance.

​

  • Residents should be able to self administer medications, with help of health literacy support staff and ancillary team.

​

  • Medications are stored in locked secure setting and to be received under staff supervision.

​

  • On-site staff supervision is provided 24/7.
